What You’ll Do Conduct face-to-face assessments with members to evaluate medical needs and overall wellness. Help members access vital services such as: Adult & Pediatric Medical Daycare Personal Care Assistance Nursing Facility Custodial Care Personal Preference Program MLTSS Program Enrollment Complete 20 assessments per week (based on need) using a holistic approach that considers co-morbidities, multiple diagnoses, and functional limitations. Collaborate with your team and supervisor to overcome barriers and achieve goals. Use your interviewing skills to engage members and uncover their health needs. Mentor new team members once you’ve mastered your role. Operate at full capacity by month four with comprehensive training and support. What We’re Looking For Active, unrestricted RN license Residence in Essex County, New Jersey . 2 years clinical experience (hospital, home health, or ambulatory care). Reliable transportation and willingness to travel up to 75%. Comfortable working with diverse populations in various settings. Strong attention to detail, organizational skills, and ability to work independently. Computer proficiency and experience handling medical records. Ability to work Monday–Friday, 8:00 AM–5:00 PM EST . Preferred Skills: Assessment, case management, or discharge planning experience. Managed care or crisis intervention background. NJ Choice certification (or willingness to obtain). Strong interviewing and communication skills. Proficiency with MS Office Suite and EMR systems. Education Associate Degree in Nursing required ; BSN preferred. Anticipated Weekly Hours 40 Time Type Full time Pay Range The typical pay range for this role is: $66,575.00 - $142,576.00 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above. Our people fuel our future.
