Job Description Company Description: Crossroads was founded on May 5, 2005 to serve a patient population battling with opioid addiction. Since its founding in 2005, we have been a physician led healthcare organization. The company has grown rapidly and now includes 90 treatment centers in 10 states. Each member of the Crossroads team specializes in an area that supports the recovery of over 26,000 patients, monthly. Crossroads' clinical staff includes physicians, pharmacists, counselors, nurses, and other service coordinators. Along with medication management, our staff works closely with each patient to create an individualized treatment plan aimed at building recovery and relapse prevention skills. Job Summary: We are seeking a compassionate and strong Patient Navigator to join our team for our OBAT (Office-Based Addiction Treatment Program). This position serves as a single point of contact for referring physicians, patients, and caregivers to provide resources and assistance with accessing clinical and supportive care services offered within Crossroads Treatment Centers and in the community as well as facilitating patient appointments. Travel is required in this position to most of our North New Jersey clinics. Locations include: Belvidere, Stanhope, and Hamburg. Mileage reimbursement applies, speak to recruiter for details. Essential Duties and Responsibilities include the following and other duties that are assigned: Greeting patients and answering clinic phones. Assist patients in understanding their treatment options, and the resources. Serve as an essential link between patients and all other care. Facilitate appointments for consults and support services within established service standards, including social workers, physician, etc. Coordinate multidisciplinary planning conferences, develop concise patient summaries for use by the care team, and document recommendations made utilizing standardized care protocols in accordance with nationally recognized care guidelines. Follow patients throughout the care continuum, including inpatient admissions, and collaborate with inpatient care management. Work with staff and others to collect data, track outcomes, and support strategic planning processes. Adhere to established productivity. Responsible for outreach efforts to establish and maintain positive working relationships with physicians, office staff, nurses, social services. Assist Center in stocking supplies. Position Requirements: The most qualified candidates will possess the following: Certified Medical Assistant (CMA) or Associate or Bachelor's degree in related field. Requires excellent leadership skills, as well as excellent organizational, written, and verbal communication skills. Must be able to work in a self-directed environment, with an ability to work with and lead teams.
