We are currently seeking a Full Time Case Manager for our St. Bridget's Residence Shelter , located in Newark, NJ . Job Duties: Responsible in assisting clients with the appropriate service referrals as well as developing a service plan to enable clients to meet their potential goals. Linking individual clients to services with program activities that suitable for clients needs. Responsible for continuity of care and assuring clients are maintaining productivity.
