SUMMARY OF JOB:
Assists in meeting the psychosocial needs of patients/families and enables patients to utilize health care in order to achieve their optimal level of health. -CONTINUING OUR MISSION -Redeemer Home Care and Hospice's ability to continue the rich heritage of the Sisters of the Redeemer and care, comfort, and -heal -in the home is directly related to the talents, motivation and accomplishments of our employees. - A primary way staff connect to our mission is by collaborating with all team members in promoting an environment that provides quality, compassionate, holistic, patient-centered care and supports all internal and external customers. -

RECRUITMENT REQUIREMENTS:

• Master's degree from an accredited school of social work.
• Current NJ license required
• 1-2 years Post graduate degree, experience n a health care setting, (hospital/home care preferred).
• Flexible, sound judgment, customer oriented, organized, keeps professional demeanor at all times.
• Excellent verbal and written communication skills.
• Available to work flexible hours, with weekends and holidays included.
• Knowledge of general disease process.
• Knowledge of family dynamics and interpersonal relationships.
• Member of the Academy of Certified Social Workers, signifying two years of practice under qualified supervision preferred.
• Able to travel to patient's home within service area.
• Able to use office equipment; i.e. beeper, car phone.
• Valid driver's license.EOE
