Serenity Hospice Care - *****REGISTERED NURSE - RN - CASE MANAGER - HOSPICE - Essex/Union Counties: -Serenity Hospice Care is looking to hire a 'Registered Nurse (RN) Case Manager -Team Lead -to join our team of professionals for Essex/Union Counties.
Registered Nurse RN Job duties can include, but are not limited to:

• 
  
• Orienting new RN's.
  
• Assisting with RN questions and needs.
  
• Preparing for bi weekly team meetings
  
• Running Bi Weekly Team Meetings
  
• Ensuring that best practices are always being adhered to.
  
• Scheduling orientations for new RN's.
  
• Completing Competencies and Performance Evaluations
  
• Meeting with employees for review and putting necessary plans of correction into effect.
  
• Reviewing all RN documnetation
  
• Reviewing all CHHA Care Plans
  
• Counsels the patient and family in meeting end of life needs
  
• Initiates appropriate preventative nursing procedures
  
• Registered Nurse Supervises the Hospice Aides and LPN's
  
• Must be very knowledgable with Medicare Regulations
  
• Must have a firm understanding of appropriate Hospice Diagnosis
  
• Effective communication and relationship building skills
  
  The proper candidate must have a current New Jersey RN Registered Nurse License in good standing, must be familiar with and willing to travel the area . Must also have reliable transportation and insurance. Monday through Friday 8:30am to 4:30pm. Care for Hospice patients in their homes and facilities.
