SM This is a per diem position. You will be--expected to be available at least 24 hours/month and 16 of those hours should be during off-hours (evenings/weekends). This position would be covering LTC facilities in the--Newark NJ Area. $2500 Sign On Bonus for external candidates! In this role, you will represent Optum as you provide primary care to patients in long term care settings; coordinate with their Primary Care Provider and facility staff to deliver high quality care on site. You'll interact closely with the facility staff and providers throughout the community; educating, building relationships and showing leadership in how to best address the complex needs of the patient and family. This is a flexible, autonomous role that creates enormous satisfaction for the NP as you impact the care and comfort of our aging population. Primary Responsibilities

• Develop strong competencies in Palliative Care as you provide preventative and primary care for patients in assigned long term care facilities
• Collaborate with primary care physicians, the nursing staff and the patients' families to coordinate care in the manner best suited to the patient's needs Consider this.--Success in the NP role requires learning how to strike a balance between clinical and customer service / business mindset. Your work within these independently owned / operated long term care facilities means that you represent us not just with our members and their families but also with the employees and leaders of each facility. Balancing how to work with, educate, and service the facilities staff while also providing the members with the clinical and caring attention can be a challenge. You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. Required Qualifications
  • Current, unrestricted NP license in the state of assignment
  • Ability to obtain DEA licensure / Prescriptive Authority post - hire in states where applicable
  • Clinical background and national certification as ANP, FNP, GNP--required
  • Past experience working in a nursing home or with seniors in other settings
  • Intermediate level of proficiency in PC based word processing including Microsoft Word and Outlook
  • Employees are required to screen for symptoms using the ProtectWell mobile app, Interactive Voice Response (i.e., entering your symptoms via phone system) or a similar UnitedHealth Group-approved symptom screener prior to entering the work site each day, in order to keep our work sites safe. Employees must comply with any state and local masking orders. In addition, when in a UnitedHealth Group building, employees are expected to wear a mask in areas where physical distancing cannot be attained
  • You will be provisioned with appropriate Personal Protective Equipment (PPE) and are required to perform this role with patients and members on site, as this is an essential function of this role.
