Job Description:

The System Executive Director, Case Management oversees AHS s case management program, ensuring best-practice processes and standards are implemented and generate successful outcomes at all AHS hospitals. The System Executive Director, Case Management is responsible for planning, directing, and supervising all aspects of the case management departments, including clinical case management, utilization management, and clinical social work. In addition, the System Executive Director of Case Management acts as a liaison with other departments throughout the system to better facilitate resource utilization, communication, and collaboration among all care partners. The System Executive Director, Case Management plays a pivotal role in achieving AHS s operational and strategic goals through performance improvement, collaboration, communication, and education. To be successful, the System Executive Director, Case Management should demonstrate excellent communication skills, strategic and innovative vision, proven motivational and change management capabilities, and a demonstrated track record of delivering results and achieving goals. Responsibilities

  • Provide direct leadership, support, oversight, and accountability for the operational efficiency and effectiveness of Case Management at the System and hospital level.
  • Collaborate with the case management leadership and other system and hospital leaders (Presidents, CFOs, CMOs, COOs, CNOs, Quality, Legal) to develop System case management goals to improve operational, clinical, quality, financial, quality, and patient experience outcomes.
  • Collaborate with case management, system, and hospital leaders to develop and execute case management strategic plan and priorities, including infrastructure and technology investments and adoption of innovative technologies to improve efficiency and outcomes.
  • Proactively analyze data and identify opportunities to optimize performance, build competencies, and create a more rigorous approach to case management, with a focus on throughput, resource utilization, addressing psychosocial needs, and sustainable transitions.
  • Partner with patient financial services, contracting, and physician advisor teams to achieve efficient, effective utilization review processes and outcomes.
  • Collaborate with nursing, bed management, medical staff, and other stakeholders to prioritize throughput and eliminate discharge delays to achieve System targets.
  • Ensure case management metrics and performance are standardized, align with, and support System priorities in revenue cycle, patient flow, and population health.
  • Coordinate Conditions of Participation and CMS regulatory audit programs associated with reviews of patient records to determine necessity and appropriateness of care rendered to patients.
  • Provide direction and manage hospital case manager leaders to ensure standardization and consistency across the health system.
  • Develop a culture of high performance and continuous improvement that values learning and a commitment to quality.
