Job Description:

Managed Care RN Field Case Manager
Responsibilities:

Travel to members’ homes for comprehensive assessments
Travel personal car up to seventy-five percent
Develop a Care Plan
Authorize services
Coordinating care with providers and members
Schedule and attend interdisciplinary meetings

Requirements:
- Minimum of 3 years clinical practice
- RN License Sate of NJ
- COVID Vaccinations required
- Strong writing and communication skills
- Ability to learn and utilize technology
- Case Management preferred
- Managed care experience preferred
- Home care experience a plus
